Improve the search algorithm
The search needs to be smarter and prioritized. Example: If you search "Secret of My Success", the Michael J. Fox movie is not returned because one of the S's is a dollar sign. With only one character off, the algorithm should return the movie as a result.

Create a tag for short films
Having a separate "medatype:short" tag, instead of grouping all shorts with feature films under "mediatype:movie", would often make searching/filtering easier.

Add the ability to compare watchlists with a friend and display movies you both want to see but haven't checked.
This would be very helpful for picking films for a movie night with friends - personally I would find this more useful than being able to compare checked movies.
Preferably, the same feature could be applied to recommended movies.

Add a 'Choose a random movie' button
This button returns a random unchecked movie which is listed on one of the official toplist.

Make it possible to create movie checking goals
The idea is basically to have a simple goal of how many movies you aim to check (similar to the reading goal on Goodreads). This could be on a yearly or monthly basis.
I think the idea should be as simple as possible, but you could make it more customizable, too. I'd say you should only include feature films, but you could allow yourself other types. Or you could make it "in <x official lists" or something similar.
In any case, it works well for books and would presumably be a relatively small feature to introduce.

When you watch a movie on your watchlist, it should be automatically removed once you check it
As of now, "Remove from Watchlist" has to be clicked separately, in addition to checking it

Settings to Edit the Main-Page (suggest movie at first page etc.)
I will always see this message at main-page
"Need more friends? You can find your online friends from other sites or you can add friends manually. "
bcs I didnt add any friends I dont use ICM to find my friends or see what movie they like I just want to discover new movies.
So I would like to have a personal settings for main-page, like a 'random movie' or 'movie suggestion list' instead of "Here's what your friends have been up to lately".

Make it possible, when comparing profiles, to look at another user's progress on unofficial lists, not just official ones.
Not each of the thousands of unofficial lists, but have a button on the user's profile page "view his/her progress on your favorite/watchlist lists".

Make recommendations based on lists not neighbours
Instead of using neighbours to generate favourites, figure out which lists I like and make recommendations based on those.

Recommend films from a selction of lists
Allow user to highligh a number of lists they wish to make progress on and then ask icm to recommend films. The site would then return recomendations favoring films that appear in the most of the users chosen lists.

Rank Personal List By Choice (like favourite movie ranking)
I have created around 30 Personal Lists and 3 of them I use to organize my checked films, which means I update them every time I watch a movie. It is really aggravating having to search through my personal lists looking for a specific one.

Allow more than one alternative title for films
Many foreign films are known by more than one name in English. Example Bergman's 'Gycklarnas afton' has the aka 'The Naked Night' but it is widely known as 'Sawdust and Tinsel'.

Differentiate between profile page and dashboard page
Currently the profile page is a mixture between what others may find interesting and what I find interesting. I suggest creating a page that only you can see and is more adaptable to what you want to see when you login yourself. For example, I don't want to see my own activity stream, lists, favorites, owned, settings or awards block every time I log in. I would however like to see my friends activity, stats, checks.

More official lists!
I find I often don't watch films because they are not on official watch lists, important films that are on many great lists that have not been made official yet. I hate letting this control my viewing and have been trying to fight it, watching films that are not official and then find that I'm falling down the chart. I just think that a real lover of cinema should not be limited to so few films. So, MORE official lists!!! It is the lists that brought us to this site, not social features, even though they are nice also. :)

Separate useless personal lists from quality ones
There are ~2500 personal lists on ICM now which are interesting only to their creators, e.g. "Watched in 2011", "Watchlist", "My favourite films" and so on. People created them because they don't know how to use specialized ICM features for such tasks (Owned movies. Watchlist)
Several measures that can reduce their number or hide them completely:
1. Appoint moderators from power users who would regulate tags of personal lists and put useless ones in distinct hideable group.
2. Add privacy option to lists to access only by link.

Allow for 4 or 5 star ranking system.
Instead of "like" "Dislike" or leaving it blank, I wouldn't mind a system that allows me to rate a movie after I watched it. It would also be interesting to see what movie has the highest "star" and lowest "star" rating.

go back to the previous compare profile feature
In ICM 1.0, when you wanted to compare your profile to a friends, you had all the lists (with the icons) subdivised in 2 bars, one for your self, one for you friend. It was much clearer than the actual setting "You lose by X checks", to my opinion.

Integrate Streaming Services
If a movie is available on any streaming service (Amazon, Crackle, Hulu, Netflix, etc.) provide a button on the movie's page to watch it on the service of the user's choosing.

Fix the quick search 'suggestions'
It does not work well.
Try searching for "Ong-bak" and you will get zero quick suggestions. Try searching for "ong bak" and you'll get "Ong-bak (2003)" as suggestion #1 and "Bus Stop (1956)" as suggestion #2.
